- What City and Province are you coming from?
- Have you been directed to self isolate by a public health official in the past two weeks?
- Have you been in close contact with anyone who is lab confirmed COVID-19 positive?
- Please provide all names and phone numbers of those requesting approval to enter Ahousaht.
- Why are you requesting to enter Ahousaht?
- what organization are you from/representing?
- What date are you requesting?
- You are required to provide your own Personal Protective Equipment (Mask, gloves, and sanitizer)
- Do you and the group/family you’re traveling with have COVID-19 Symptoms?
• fever
• dry cough
• tiredness
• aches and pains
• sore throat
• diarrhea
• conjunctivitis (pink eye, infection of the eyes)
• headaches
• loss of taste and/or smell
• rash on skin, or discolouration of fingers or toes
• difficulty breathing and/or shortness of breath
• chest pain and/or pressure
• loss of speech and/or movement
